For the 2023-24 school year, an MHSAA pre-participation sports physical is required for all students who participate in sports. A physical examination must be completed on or after April 15, 2023 to be considered a valid physical for 2023-24. A STUDENT SHALL NOT PARTICIPATE IN ANY PRACTICE SESSION OR CONTEST UNTIL THIS ONLINE REGISTRATION FORM HAS BEEN COMPLETED.  If the student does not have an up-to-date physical (one on or after April 15, 2023), they will be required to have one uploaded on FamilyID with completion of the online registration process or turned into the Athletic/Middle School Office before participating.  The MHSAA Sports Physical form can be found at: https://www.mhsaa.com/portals/0/Documents/health%20safety/physical2page.pdf.
