For the 2020-21 School Year, Swan Valley Middle and High School will use an online registration and payment platform called FamilyID.  With the uncertainty of fall sports because of Covid-19, athletic registration through FamilyID is scheduled to open up on the Swan Valley School District web page at (Go to Menu; Athletics; FamilyID Sports Registration and Fee) on August 1, 2020.

By completing the online sports registration, both student-athletes and parents give their consent to all items found in the Athletic Handbook including pay-to-participate information, transportation, emergency medical authorization, physical requirements, concussion information, equipment responsibility, all health warnings, and the Athletic Code of Conduct.  

The fee for the 2020-21 school year for athletes in 9th through 12th grade will be $100.  The fee for the 2020-21 school year for middle school athletes will be $50.  If an athlete qualifies for free or reduced lunch, all fees will be reduced by 50%.  There will be a family cap of $250. Self-funded sports are excluded from the fee (Hockey, Boys/Girls Bowling, Sideline Cheer, Equestrian, Middle School Cross Country, Middle School Wrestling).   If preferred, you may also pay by check or money order to: Swan Valley School District.  If not making not an online payment through FamilyID, please turn in all checks, money orders, or cash to the High School Athletic Office.

Just a reminder for the 2020-21 School Year, the MHSAA pre-participation sports physical requirement will be waived for students in any grade who received a valid physical for sports during the 2019-20 school year (one completed on or after April 15, 2019). These students and their parents/guardians will be required to complete the two-page MHSAA ANNUAL SPORTS HEALTH QUESTIONNAIRE found in the High School Athletic Office, Middle School Main Office,, or (Go to Menu; Athletics, Sports Health Questionnaire / Physical Form).  This must be signed by the student and parent/guardian and kept on file in the school.  The FamilyID online sports registration will also contain the questionnaire.
